Hi all. I have a 2003 with 8.1 volvos and went through all the procedures mentioned above, then I found out that there was an issue with the paint on the inside of the fuel pump and during the ethanol days the paint came off and caused this problem. I contacted the "man Cave mechanic and got an education and problem solved! https://mancavemechanic.com/

Take a look at this post on my experience of a similar issues. It is probably a good time to repost it as it has been a few years. It was quite perplexing at first and then upon further research it is not all that uncommon with that vintage gas fuel pumps.
