Hi all. I have a 2003 with 8.1 volvos and went through all the procedures mentioned above, then I found out that there was an issue with the paint on the inside of the fuel pump and during the ethanol days the paint came off and caused this problem. Take a look at this post on my experience of a similar issues. It is probably a good time to repost it as it has been a few years. It was quite perplexing at first and then upon further research it is not all that uncommon with that vintage gas fuel pumps.
